Create a pre-designed Web form

  1. On the Objects toolbar, click Design Gallery Object Button image.
  2. Under Categories, click Reply Forms.
  3. Under Reply Forms, choose a form.
  4. Click Insert Object.
  5. Double-click each control and select the options you want.
