Bill individual, family or agency

1. Individual  - the patient
2. Family  - the head of the patient's family, if the patient belongs to a family  
3. Agency  - an Agency  

The bill can come from either the doctor or the practice.

When billing to an agency

select the Agency Code from the drop down list. 
Select the Fee Type from the drop down list.
Insert a Claim number if this is applicable.
